DIP Team Nurse Referral Pathway
Non-Urgent Referral Criteria - Instruct client to follow-up with their physician
- Blood pressure: 130/80 Hg
- Blood glucose: Screening value > 7.0 mmol/L
- Microalbuminuria: ACR > 2.0 Males mg/mmol and ACR > 2.8 Females mg/mmol
- Foot Assessment: Evidence of neuropathy
Urgent Referral Criteria - Immediate referral to physician / specialist
- Blood Glucose > 20.0 and experiencing any of the following: Dehydration, abdominal pain, vomiting, fruity smelling breath, weakness, decreased level of consciousness
- Blood Pressure: 160/100 Hg
- Experiencing headache, vision changes or dizziness
- Foot Assessment: Evidence of infection (redness, warmth, swelling, pain, ulceration, or gangrene)
