Province Wide Coordinator - Full Time Term
(April 1, 2010 - March 1, 2011) - Winnipeg, MB
Job Summary:
The Province Wide Coordinator will be responsible to assist in the implementation of the Diabetes Integration Project program components and work collectively to enhance the capacity development activities of the Mobile Diabetes Health Care Service Delivery Teams and to assist with the coordination of all province wide activities. The Province Wide Coordinator will report directly to the Executive Director and will provide functional supervision to the Mobile Health Care Service Delivery Teams.
Activities to achieve objectives will include but not limited to:
- Conduct Orientation Sessions and Capacity Development activities for the Mobile Diabetes Health Care Teams
- Assist in the development and/or revision of policies and clinical procedures required for the Point of Care Testing, all Nursing Assessments, Clinical Care Pathways, Emotional Wellness, Foot Care Assessments/Treatment, and others as required.
- Establish a partnership and network with the First Nations communities, Regional Health Authorities, Northern Medical Unit and Manitoba Health for the provision of Mobile Diabetes Care and Treatment Services
- Work collectively with the Health Care professionals at the First Nation community level
- Assist in the management of Quality Assurance activities such as chart audits, nursing activity reviews, case reviews, keep abreast of morbidity and mortality reports, performance appraisals, and opportunities for continuing education
- Assist in the development and implementation of training programs to build capacity in First Nations communities and certify diabetes workers as required.
Qualifications/Education/Experience:
- Completion of a Baccalaureate Nursing Degree or RN with five years relevant experience from a recognized educational institution and current registration with the College of Registered Nurses in Manitoba.
- Certified Diabetes Educator designation with expertise in the application of the 2008 Canadian Diabetes Association Clinical Practice Guidelines
- Experience with development of information technology and database information systems
- Experience in the application of the Principles of Teaching/Learning Programs
- Proficient in the application of various computer software programs
- Must possess excellent verbal/written communication skills, the ability to work collectively as a team member and ability to foster a positive working environment
- Must have valid Manitoba Drivers License
- Knowledge of First Nation customs, values and traditions
